Who We Serve

We support individuals with intellectual and developmental disabilities and autism across Pennsylvania, on the Consolidated, Community Living (CLW), or Person/Family Directed Support (PFDS) waivers. We specialize in medically and behaviorally complex individuals who need Enhanced IHCS, 24/7 staffing, or skilled in-home nursing.

For Supports Coordinators

We’re a responsive HCBS partner with a direct line to leadership and quick turnaround on referrals. We’re fully PA ODP enrolled, with documentation and billing through PROMISe. Send the individual’s information and waiver details and we’ll coordinate next steps promptly.

  • T2025 TD – Shift Nursing, RN (1:1)
  • T2025 TE – Shift Nursing, LPN (1:1)

Waivers Accepted

  • Consolidated – uncapped
  • Community Living (CLW) – $97K/yr
  • Person/Family Directed Support (PFDS) – $47K/yr
