Introduction

Caring Hands Loving Hearts provides a full range of home and community-based services for individuals with intellectual and developmental disabilities and autism across Pennsylvania. We accept the Consolidated, Community Living (CLW), and Person/Family Directed Support (PFDS) waivers, and we work closely with individuals, families, and Supports Coordinators to deliver care that fits each person’s life and goals.

We specialize in medically and behaviorally complex cases – including individuals who need Enhanced IHCS, 24/7 staffing, or skilled in-home nursing.

Our Services

We offer a comprehensive suite of home and community-based services tailored to each individual’s needs.

In-Home & Community Support (IHCS)

Person-centered help with daily living and meaningful community participation. IHCS supports individuals in building skills, maintaining independence, and staying connected – at home and out in the community. Available in standard and enhanced tiers, with 1:1 and group ratios to match each person’s needs.

Companion Services

Non-medical companionship and supervision that helps individuals stay engaged, safe, and connected during everyday activities – at home, on errands, and in the community.

Respite Care

Family caregivers do extraordinary work, and they need rest too. Our respite services provide dependable, compassionate relief – short-term or scheduled – so families can recharge knowing their loved one is well cared for.

Shift Nursing (RN / LPN)

Skilled nursing care delivered in the home by licensed Registered Nurses and Licensed Practical Nurses, for individuals with complex medical needs. Both RN and LPN tiers are available, with differentials for high-acuity care.

Family Medical Support Assistance (FMSA)

Support for families caring for a loved one with complex medical needs. FMSA handles the coordination – medical appointments, physician communication, pharmacy refills, and care documentation – with a licensed nurse providing clinical assessments and hands-on training for staff and family caregivers.
